I've struggled for years and been told I was a every unhappy child when I was 12/13 when forced to the doctors and I've perfected hiding everything with a fake smile but I've done tests online and they've been very concerning and I've been to scared to contact my local gp as I have no idea how to explain my issues and feel like I'm over reacting

Hi, I am so sorry that you've had a hard time and not received the right help at the right time. An unhappy childhood can lead to problems in adult life. There are often underlying causes to the the unhappiness in one's childhood, they could be external or internal. We often internalise how we feel and that an lead to anxiety and depression. You are not over reacting, maybe just scared? I would like to assure you that if you feel something is not ok then its a very good possibility that you're right. Pls reach out to me here if you feel I can help else anyone who is a counsellor or psychologist whom you connect to and feel can help you. You do need support and its a good idea to reach out for help. Someone new and unbiased is a good option. In the meantime, try and make a list of all your symptoms and emotions and feeling , if you pen it down, it helps release the initial emotional distress. I find walking also clears the mind. Hope you feel better soon.
