the diet has to be personalised and it will be difficult to give a diet chart based on the information you have given. I suggest that you go through the website called kidney.org of the national kidney foundation of USA to get a broad outline about your diet.

hey, for diet chart for complicated cases you have to consult dietician personally,because it has to be prescribed after detail check up and investigation to know at what stage ckd and dm are there he has to restrict proteins , sugars.
