Detailed history and investigation is needed. .But as I can understand its mild disc bulge at L3-L4 nd L4-5..Which is not the cause of your discomfort. .It's myofascial triggers which is referring pain towards your toes and giving u night cramps ...
Next Steps
plz get it released by  a sports physio with expertise in myofascial trigger points release therapy (manually or dry needling)..of following muscles ...q.lumborum , latissimus dorsi,g.meduius MINIMUs and maximus ..piriformis ..rectus femoris ,TFL,.gastrosoleus ,..tibialis anterior ..

If you have back pain associated with pains going down the leg you will probably have a disc bulge or prolapse putting pressure on the nerve. You need to see a Spinal Surgeon for assessment and investigations as appropriate. Once you are assessed you may need an MRI scan even if you have had one in the past as your symptoms have changed. You may need directed physiotherapy, painkillers, yoga, posture improvement and hot fomentation. Please see a Spinal Surgeon. Hope this helps. In my opinion u need to go for thorough check up like few blood test like calcium vitamin d vitamin b 12 uric acid thyroid hba1c with blood sugar...these pains can be becoz of the deficiencies of these components in body...after this visit a Orthopedics followed by few sessions of physiotherapy then u will feel the difference as need it very badly... get well soon take care
Health Tips
If blood tests get a deranged value kindly take the medications to meet the derangement Complete the course of them in a serious manner Do not sit for too long and do not stand for too long Postural correction is needed Have plenty of water atleast 3 to 4 ltrs a day Have plenty of fruits and green leafy vegetables with protein intake
