Do wearing headphones (all types) for longer time causes wax formation inside ears?
Also does wearing earphones increases bacteria formation inside ears?
If so, what we can do to prevent it while using earphones?

Usually headphone and earphone usage won't cause wax accumulation or infection. Normal wax inside the canal itself prevents infection of the ear canal. No specific prevention is required. You should avoid using ear buds
