The normal liver size in ultrasound is 15 to 16 cm in the mid clavicular line, which is almost in the middle of the front of right chest. By ultrasound standards your mom has hepatomegaly or increase in size of the liver. This is not a diagnosis of a disease but only an USG finding which may or may not indicate a liver disease. Diabetic patients suffer from fatty infiltration of the liver which can be graded and which alters the liver echo texture. In all probability your mom may be suffering from a fatty liver for which you need not worry about, much. However there is also a link between fatty liver and liver cirrhosis which is a serious disease. Drug induced cirrhosis occurs. You haven't mentioned the drugs your mom is taking for diabetes mellitus and high blood pressure.
Next Steps
We don't treat reports, we treat the patient. So, just a hepatomegaly in ultrasound with change in liver echo texture doesn't provide us with much information except that we have to be cautious.
Health Tips
Do the following investigations : 1. Liver fibroscan, which will exclude cirrhosis by assessing the amount of fibrosis. 2. Do a repeat ultrasound by a specialist hepatobiliary sonologist to exclude a rise in the blood pressure of the intra abdominal portal venous system which carries blood from intestines to the liver. A high pressure ( portal hypertension ) almost clinches the diagnosis of cirrhosis liver, in which case you have to consult a physician gastroenterologist. In any case, always avoid pain killers like diclofenac, ibuprofen ( NSAID ) and high dose paracetamol. Also show your doctor the medicines she is having. No focal liver lesions means that there is no tumor or abscess or space occupying disease in the liver. That is very assuring but sometimes we have to do a triphasic CT scan of the liver or plain CT if urea, creatinine is high. These are special Investigations which are only carried out if deemed absolutely necessary. Don't worry about these now. If further investigations advised by me are within normal limits then you may try : Tab Udiliv 300: 1 tab twice daily before meals/with meals for 3 months. IN ANY CASE, CONTACT ME ON MY MOBILE NUMBER. I NEED TO CHECK YOUR MOM CLINICALLY.
