Epidural injection for lumbar pain
My wife has got mild disk bulge and moderate disc degeneration as per her MRI report in her lower back due to wrong posture during exercise. She has been prescribed Epidural steroid injection. My concern is that she is diabetic. Is there any alternative cure apart from injection and hospitalization.

When it comes to spine problems, it is important to know what complaints the patient has. Just an MRI report is not enough. But if I were to just go by your MRI report, the problem in her spine does not seem to be major...and certainly does not reach an Epidural Injection.

Please ask her to try the following... *Hot water bag (10 minutes twice a day) *After applying hot water bag, apply Nobel Gel *Tab Flexon (1 tablet after lunch and 1 tablet after dinner for 7 days) (if she is not allergic to this tablet) *Tab Pantakind (40mg) (1 tablet before breakfast in the morning for 7 days) *Back extension exercise (Bhujangasana...start with 5 counts...then after a couple of days increase upto 10 counts...keep increasing the counts this way until you reach 30-40 counts per day) *Avoid sitting on a chair/sofa for more than 15 minutes at a stretch (get up for 1 minute and then sit again if you want) *Avoid bending forward with the knees straight *Avoid slouching back on a chair (sit erect) *Avoid riding bike/car as much as possible *Avoid sitting on soft sofas Do get back after doing all of the above for a week so that we can decide the future course of treatment.

Not to worry. Only  if she is over weight then she has to reduce it. What about Serum Ionic calcium  & Vit. D ? She has to give time 1/2 hr for exercises in morning & evening  -- 1/ leg raising exercises                     2/ While leg is straight and elevated to 40 degree from bed , do rotation clockwise and anti clockwise by both legs separately                     3/ lie on stomach ,keeping back upwards                        do Danushasan.While inhaling air elevate your chest and legs (keeping knee extended)                     4/ hot saline fermentation at home or to go for heat therapy by ultrasonic Ray's Control your blood sugar. If you can spare more time then go for walking ( walk like a military man , during walking weight should go toward heel and you should be erect)

The epidural steroid injection is indicated in some patients with backache, esp with radiation to the lower limbs. Case selection is based on clinical and radiological features. The epidural steroid may increase her blood sugar levels for a few days, but nothing to be worried about. Make sure her sugar levels are well controlled before the injection.

can you specify duration of her symptoms , if its of short duration .. conservative management with medicines & lifestyle modifications can be tried first along with diabetes management
