1)What are the chances of healing in a fracture shaft of humerus in a 62/f ,female if we put on hanging Cast? 2) Does the bone look like osteoporotic?

Hello dear I understand that the patient is a 62 years old female with shaft of humerus fracture. The bone quality looks ok for her age and i don't see any osteoporosis as of now. However if you are looking at cast management for her, i wouldn't agree, for there are a few problems : 1. Considering the fact she is a 62 years old female, her bone quality will deteriorate if we keep her on a cast for a long time, because of disuse. Also her shoulder will get stiff. 2. The cast might need readjustment from time to time as there is always a chance of loss of fracture reduction in plaster. 3. The hot and humid weather might also pick on her as she will have sweating and itching underneath the cast. 4. The casts are bulky and difficult to handle and might cause shoulder and neck pain I have no doubts for the fact that the patient should be operated for we can make her use her arm and shoulder early and can get rid of the plaster. However a thorough medical evaluation and fitness for surgery needs to be determined prior to that. Regards Dr Vipin Gupta

Considering the location of the fracture and pattern, an initial hanging cast followed by functional cast bracing could be a reliable treatment strategy. (However, that would also mean weekly hospital visits to ensure fracture maintains position) The bone doesn't look too porotic in my opinion, however, digital x-rays are not very good tools to predict osteoporosis.
