This appears to be a gluteal abscess. Mention if you have H/O fall or trauma, any intramuscular injection, diabetes. Start with broad-spectrum antibiotics and analgesics. It appears that you have already consulted a surgeon who has advised you for incision and drainage procedure. So better not to waste time and proceed further as it may spread more.

You need urgent  evaluation  of  gluteal pain and fever that includes  Clinic  examination by general  surgeon  base line  blood  investigations and  ultrasound  gluteal region. You have  not mentioned  any history of  intramuscular  injection  in gluteal  region. If redness  and  fever  is  there  with significant  pus it needs surgical  drainage of  gluteal  abscess

Did you have any intramuscular injection or a boil at the site of pain ? Since howmany days are you having the pain and fever ? If there is pus in the gluteal region and if it is more than 5 ml then it requires surgery which is incision and Drainage of the abscess wherein we make an opening for the first to come out naturally and restart you on antibiotics analgesics

Since how long you have been suffering from pain and fever. Usually pain and fever leads to diagnosis of infection. Need clinical examination. If there is pus them surgery is needed.

As per history ..u need clinical evaluation to come to a diagnosis If it is Gluteal abscess..then can plan for drainage of abscess as a day care procedure..

Can be a sign of gluteal abscess(pus) Show to a surgeon nearby. If it is a large abcess, incision and drainage is the best option
