You have not mentioned any history of intramuscular injection in gluteal region. If redness and fever is there with significant pus it needs surgical drainage of gluteal abscess

If there is pus in the gluteal region and if it is more than 5 ml then it requires surgery which is incision and Drainage of the abscess wherein we make an opening for the first to come out naturally and restart you on antibiotics analgesics

If it is Gluteal abscess..then can plan for drainage of abscess as a day care procedure..
