His sugar level swings very wildly
He had two recent episodes of hypoglycemia and fell down at home when his sugar level dropped to a low level of 43, as indicated by my trusted glucose meter. I want to know if there is any formula to decide on the insulin and/or Reclide dosage depending on the sugar reading.

Yes there is what is called insulin sliding scale which guides on amount of insulin for different levels of glucose.
Ask your physician to provide you one.
