Hi Doctor,
For the last few months, I am having trouble bending my left thumb. I tested positive for HLNB27 and ESR levels are above normal (44).
What does this indicate?
Kindly guide me in the right direction please.
Best regards,
Chandrima

Hello. There are 2 aspects to your problem. One is being HLAB27+ and the other is problem with left thumb. In the long run, being HLAB27+ means you may manifest certain types of inflammatory arthropathies (commonly ankylosing spondylitis) which are of familial nature. This means several/multiple joints may be affected (no need to panic/worry). You need to ensure you seek an opinion from a good rheumatologist and regularly review with them long term. When you do have bone/joint problems, the rheumatologist will ensure you are on appropriate medical treatment which is key to preventing a lot of long term complications. Also, please make a healthy lifestyle (good posture, optimum weight & exercises to keep supple joints) a part of who you are.
Next, your current problem with thumb: For these specific problems which you may face from time to time, you need to see a hands/orthopaedic surgeon fr specific treatment. It is important that the doctors you see (both orthopaedic & rheumatologist) are committed to communicating with each other to ensure your best interest as a patient. So choose carefully.
Hope this helps. Take care. Dr M Vashishtha FRCS
