Hello, now-a-days one of the most common cause of all these pains are "Myofascial pain", this is more common in females, and in most of the cases this is having a postural component also. Only physiotherapy is not a solution for your problem, consult some Physiatrist/posture specialist who will be able to guide you in a better way.

Go to a good physiotherapist for proper diagnosis. Shoulder and hand pain may related to your cervical spine problem. Don't try to decrease your symptoms of pain, try to find out reason for that and treat that. That will automatically relieve your pain.
Next Steps
Do shoulder girdle exercise. Do pectoralis major muscle stretching. maintain your posture.
Health Tips
apply hot water bag twice daily for 15 mins at painful region.
