Seems you have grossly decayed tooth with signs of pulpits and infection. If the tooth is beyond repair it has to be extracted. If it is not that badly damaged you can save it by root canal treatment followed by cap. If it is your wisdom tooth go for extraction. Usually it will be done under local anesthesia hence don't worry about pain though you will have mild discomfort for 24hrs to 48 hrs depending upon your pain threshold. Normal extraction would cost around 1000-1500, if it's removal of wisdom tooth it may cost little more.
Next Steps
Consult a dentist to get immediate pain management and get the procedure
Visit ur dentist to evaluate the problem with clinical look or upload a picture here on practo and if possible get it treated with RCT don't extract ur natural tooth.
