Medical Fitness Certificate

Asked for Male, 35 Years

I need a medical fitness certificate for joining a private company. i have got all the tests done as specified in the tests to be taken. Who should i approach to issue me the fitness certificate? i have the test reports with me. only pening is ecg and xray. I shall gey it done when i visit a doctor for fitness certificate. please help me how can i get the fitness certificate
