I have problem to remind name . concentration problem.I can forget what the name I have listen before 5min. Some time it takes 10sec to remind very familiar word.

It can be just simple in attention and nothing else that's why you having problems. Consult a neurologist or a Psychiatrist if you are not able to memorise even after revising and recalling things again and again.
