Needs clinical examination . If there is no weakness in the legs / urinary disturbance , then surgery can be avoided.

This is a incomplete report, ask for AP spinal canal diameters at disc level. share xray too -Lumbosacral Spine - Lateral (standing).

Need actual films and xrays to know more . If conservative trial has failed you may require surgery or can try root block in such cases, also detailed history is needed
