The first thing that you need to do is good the Habit immediately because it has started to show the damaging effects which are very very dangerous. The next step should be to go for a proper clinical evaluation by a dentist, preferably an oral pathologist, so that the diagnosis can be established and the right treatment can be suggested accordingly.
The most likely diagnosis based on your description is Oral Submucous Fibrosis and it can progress to oral cancer if the Habit continues unabated.
