Impression on MRI of left ankle : Old fracture of anterior aspect of the lateral malleolus with no bony bridging. Irregular contour of the anterior talofibular ligament likely old high grade sprain. Moderate chronic tendinopathy of juxtamalleolar and inframalleolar peroneus longus tendon ; no tear. Mild tendinopathy of Achilles tendon; no tear

Well, it's an old fracture, but what's the problem. I mean are you having pain with normal activities or with only strenuous activities. What are you expecting ( do you want pain free ankle while walking or you want to participate in athletic activities). Depending on your reply i can advise
