According to details you have mentioned it seems that
1. post paralysis effect is still present, and muscles are getting weaker.
2. You are not using your affected body part so losing strength
Can you please elaborate your condition, like
1.side of body affected-left or right
2.Gripping of objects is the problem or you have pain while lifting things on shoulder??
3.Is recovery complete after paralysis?
This is due to muscle weakness. Since it is a neurological case, there might be patterned movements and loss of strength. It is advisable to start with strengthening of shoulder, elbow and upper back (scapular muscles)musles. Start with 0.5-1kg dumbbell or weight cuff and use of theraband (medium resistance)for exercising.
Muscles to be strengthened
-biceps-dumbbell
-triceps-dumbbell
-scapular muscles - theraband , push ups in standing against a wall.
