Hello sir! Prolonged pain or sensitivity after exposure to cold indicates pulpal inflammation which is called as pulpitis. It is mainly caused by bacterial infection which itself is a secondary development of caries. It can be reversible or irreversible. Reversible condition can be treated by  removing the cavity and filling the tooth. For irreversible pulpits RCT can be done.
