Penis cover not opening

Asked for Male, 42 Years

Since 1 year and more, the cover of the penis is not opening. Whenever I try to open it, the cover skin gets multiple cuts and it's very painful. This started an year back. At the tip the skin has become very dry and rough. Seeking suggestion from the experts. I am 42 years.
