Bad breath or Halitosis can be due to various reasons.  As your informing us that the Dentist already told you that you have a good oral hygiene,  the reason must be something else which needs to be diagnosed. Bad breath can also be due to Gastric problems, Liver disorders, problems related to kidneys, Sinusitis, Lung infections, Diabetes, Dryness in the mouth, Starvation  (not taking food for long), Smoking/ Tobacco chewing.
Next Steps
If you have any if the mentioned problems like gastric or sinusitis kindly visit the required specialist and try to rule out the problems so that you can know what the actual cause is.

Hello, bad breath could be because of deposits on teeth surface or food lodged in between teeth. Visit a dentist and get your scaling and polishing done. Brush your teeth twice daily, night brushing is very important. Floss regularly. Rinse after every meal. Visit dentist every 6 months.

It's a wrong myth that only oral cavity can cause bad breath, frequent gastric irritation, common cold, sinusitis and some medications can cause dry mouth and eventually bad breath or halitosis.
Next Steps
If your dental hygiene is good, you can rule out other possibilities as well by consulting a physician.
Health Tips
Keep yourself self well hydrated, moist mouth will reduce bad breath to an extent or chew sugarless chewing gum that stimulates salivation.
