You have multiple problems occurring concurrently which may or may not be interdependent. Extraction of a wisdom tooth is followed by a healing phase which can cause some discomfort to the patient but should be over by this time. But if the socket has not healed completely it can be reason for discomfort in that area and also a potential source of new infection. This will be corrected by some objective treatment done by the dentist and this treatment will depend upon the current situation of the extraction socket based on a proper clinical examination. As regards your tobacco usage, if you are even using it occasionally you cannot say that you have quit the habit. You seem educated enough to understand the harmful effects of tobacco and once you are so apprehensive about the potential dangerous complications starting to occur you should now stop the Habit immediately. No doctor in the world can help you If you continue with the habit. We can only make the diagnosis and establish a treatment plan but if the damaging effects of tobacco continue because of its use the treatment will not show complete results. So the first thing that you need to do is visit a dentist for rectification of the problem around your healing socket and then other examinations can be done for other problems. Regards.
