If your talking about foreskin which cover the penis...it's due to increase Vata.... drying in the skin as per Ayurveda....so need to apply oleation with mahanarayana tailam which can heal old tear and soften the skin...apply in night and wash in morning
Next Steps
if the foreskin get more constricted or adhesion then need male circumcision.....
