Looking at your age, symptoms and MRI report you seem to have got a prolapsed disc at L5S1 level. However, a more elaborate history and physical examination is crucial to make a decision.
Important points in history:
1. How did you develop this problem- lifted any heavy weight or sudden bending?
2. How long have you had this pain?
3. Do you have any tingling sensation over calf or feet?
4. Have you developed any weakness in your ankle or foot ?
5. What treatment have you undergone uptil now and has it benifitted you?
Additionally i need to have a look at the MRI films because the report is just a depiction and doesn't reliably quantify the problem.
To conclude you can either be put on an exercise regimen plus medicines or might need a surgery if need be and all other measures are exhausted.
