Skin patches with itching on lower back

Asked for Male, 28 Years

I am having some rough patches at center of lower back from one week and now it starts paining . Can someone please tell me what it could be and what medication I need to follow or do I need to consult a dermatologist ? Please find attached image . Thanks in advance
