If a schizophrenic person (schizophrenia simplex with mostly negative symptoms and only mild positive ones which are well managed by antipsychotics) will starve for three weeks/4 weeks or more while being on medications is he at risk of getting an irreversible severe psychotic attack despite being on medication. The medication: Seroquel almost 200 mg (is respirdal more potent?)

I understand that the person you are referring to suffers from schizophrenia and is on antipsychotic medication for the same. If the person has been avoiding food or starving, it is best to check with the treating psychiatrist about the implications and consequences of the same. The psychiatrist will know the history of the person and is the best person to consult on the issue. The psychiatrist will also be able to clarify your question regarding medication. It is important that the person maintain a healthy diet which is important for overall health and well being.
