Telsartan 80 causing hyponatremia
My mother was previously consuming tazloc-H 40 twice a day. She then suffered from severe hyponatremia. The local physician stopped tazloc-H 40 and adviced cilacar-10 twice a day and metXL-25 once after lunch, but after consuming cilacar-10 her ankles and feet started swelling. So a cardiologist changed the bp tablets to telsartan-80 which is again a angiotensin ii receptor blocker(ARB). Looking at her and the symptoms I feel she is again into gradual hyponatremia. We checked her serum sodium levels it had come to 135 from 138 after consuming telsartan-80 for just 1 week. I am helpless. Please help me.

ARB (tazloc-H) to a Calcium Channel blocker (cilacar) and Beta Blocker (metXL). Then back to an ARB (telsartan). A quick remedy would be to increase the salt intake in her diet. But that may increase her BP, so this is a challenge. A good idea is to have her kidney function checked by testing her BUN and creatinine levels. Therefore in order to control her BP, it may be a good idea to switch to another BP med. Here are some choices: HCTZ 12.5 to 50 mg orally once daily, OR Benazepril 10-40 mg orally once daily (Diabetic patient, inc MAU), OR Metoprolol 50 mg orally (regular-release) twice daily (if HR is high), OR Amlodipine 2.5 mg PO QD (older patient, PVD, CAD).
