Vitiligo/ post skin grafting
I have underwent 3 plastic surgery (during 2011 to 2015) post which a thick lump of skin has gather over the grafted area also have white spots on my hand.. with uneven skin tone and skin irritation many times. I did wear skin sleeve for 5 6 years and took injections during 2018 2019 for the thick lump of skin. Kindly advice on the same.

Looks like lesion is on a mobile area like in you case elbow joint. Any injury  over the joint area,in people prone for keloid and hypertrophic scars, would end up getting them. You can keep taking intralesional injections and fractional laser to reduce it.
Next Steps
Massaging the lesion with moisturiser or silicone containing gels to reduce scar will definintely reduce the tendency of hypertrophic scar.
