What should be my next step? Help pls.
I am in USA.habe right ear,jaw pain with swelling under the ear for more than a month.went to Dr and got ant for sinus, ear infection.sinus cleared up.still the same level of jaw pain, swelling. Went to ENT and no Infection in sinus, ear. Went to dentist last week.he took x-rays and said no Infection on any tooth since I have taken antibiotics already.he did bite test and said I might have crack or fracture in low first molar.did CBCT scan.then he said looks like crack or fracture. Told me to go for extraction , implant as RCT will not be successful.i went to Endo for second opinion.he said no fracture.did cold spray test.that tooth has more cold sensitivity.so, he said I need a crown.said might have a minor crack in one spot after the bite test.i asked I have more throbbing pain in the jaw under ear and why I have swelling?he said crack will not cause swelling and dont know why. If I have more pain,he said I need RCT.he didn't take any cbct scan. What to do next?Is it jaw issue/crack?

Dear madam, The problem is in the tooth so no need to visit an ENT specialist. According to your history and x rays i can see periapical infection in your tooth. So you have to go for a root canal treatment or extraction. Crack cannot be visible in a x ray most of the times and so cannot rely on x ray. Physical tests are more appropriate in diagnosing cracks. If you want to save your teeth first undergo a root canal treatment and wait for ten days if pain completely subsides then go for a cap. If still pain persists you have to go for extraction pr else you can directly go for an extraction and replacement of teeth. Choice is yours.
