Over the past few days, I have noticed the development of small red bumps on my tongue that hurt when touched, and there is also a layer of white substance on the top of my tongue. I am concerned about my health.

The symptoms that you mentioned point to the diagnosis of hairy tongue which can be due to multiple reasons. A proper clinical evaluation by a dentist, preferably and oral pathologist will be required to ascertain the exact cause of this problem in your case. The bumps on the tongue can be normal taste buds (which are considered absolutely normal anatomical structures) or other structures which might require treatment.
