Migrain pain every month occurred

For Persistent Postural-Perceptual Dizziness I must go to which doctor I am currently doing treatment of neurologist I have a problem only one problem of unsteadiness while walking from last one and half years I also have a problem of anxiety and mild spondylitis Mild posterior disc protrusions at C4-C5 and C5-C6 levels indenting the thecal sac without cord compression.” Posterior disc protrusion at L4-L5 level indenting the thecal sac without significant neural compression.” I also go to physiotherapist they give me exercise chart for that I don't know what to do please give me answer for specific which is the route cause of my problem anxiety or pppd or spndilities So I can choose doctor accordingly
Two days ago i did a shoulder twist exercise and slept on floor and immediately i felt a pain in my neck i just slightly twisted i had a pop feeling with pain now i am feeling dizziness and lightheadedness with shoulder and neck pain i am attaching my neck xray below
I am 35 yrs old female. Under treatment for ankylosing spondylitis and seronegative arithritis for the past 2 yrs. 1 yr back I had an episode of eyebrows drooping n lips drooping, unable to move them, blood tests l l n brain MRI were done, results came negative, I was told it's bcoz of migraine. I started taking topiramate as advised. But I kept having these episodes, until this April, where along with eyebrow drooping, I had jaw locking, lips drooping, brain MRI and blood tests including were done, everything is normal. Since then everyday this would happen,generally in evening. In May another episode happened, where additionally I had temporary limbs paralysis for 6 - 8 hrs, intermittent tight squeezing of upper body, rigidity n involuntary movements of hands n toes. Chest n Abdomen CECT, AchR, RNST, b12 tests came out normal. I have been told it's not neurological, rather psychological (FND). I have started taking etizolam 0.25 mg SOS, but I find no relief in symptoms. is it FND?
From last month I have tremor in left hand thumb sometime full hand some time index finger sometime middle finger also my vitamin d is 21.1 tremor
