I am addicted to the mobile phone for the past four years continuously . I am using the phone continuously 16 hours a day. That's why I'm experiencing headache neck pain,back pain, anxiety ,hand and eye pain etc., I am unable to get over it. What should I do get rid of this addiction. I am afraid this may cause serious health problems.

Ah this will cause problems, and I'm presuming you use phone in lying position, you've to give up the habit slowly.
I'm suggesting a trick compare your screentime that'll help a lot... And then use diversion
With little tricks and some change of habits you can get away from this .
The first thing is to start physical activities like some stretching exercises , yoga or Pranayam if you don't have time then you can go for a 20 minute walk in morning & evening
These habits will improve your physical and mental health.

you can talk and discuss with your family members or friends instead of using your phone .
play physical games like bedminton or tablet tennis.
These are all just examples which distract your mind from the phone. just do it strictly for 3 weeks and it'll help
