Hi dear, it needs to be seen how much is the atrophy? Is it age appropriate or is he developing dementia. Also what kind of medications is he on and why is the aggression coming. A possibility that he has dementia and he is developing behavioural symptoms which need to be looked into. Plz see a psychiatrist in person with the reports and history so we can help him.
