Pain at nipples

He has been suffering from pain at nipples since the last two months. Having tried many home remedy but not very effective. What is the possible cure and cause of the problem. Here I am attaching a picture of his nipples

Next Steps
Try moisturizing regularly and frequently to see if helps first. If that doesn't help, further evaluation will be needed.
