Usually by 3 days after delivery good milk flow is established. As per your observation the nipples are small.....it could be due to 'retracted nipple' this needs a repeated effort from you to hold the nipple between thumb and index finger and give it an outward pull....this done repeatedly will improve the nipple to let the baby have a hold and thus suckling will be effective If you are in a metro city some baby product sellers also sell a shield like plate with central hole. If used on breast it helps make nipple come out... If your body is not producing milk its best to meet your gynecologist who can prescribe some medicines to improve milk production Sometimes stress, excess bleeding at delivery, lack of norrishment prevent good milk production too.....your gynec can help you with the treatment of these. Meanwhile Relax....keep on feeding your baby directly from breast....repeated suckling is best stimulator for milk. Drink lots of water Have good nutritious diet as calorie requirement is 500 cal higher than in pregnancy. Take good sleep...match your timetable with the baby..... Most new borns sleep during day and keep awake at night.....if the mother doesnt get rest her stress will also affect milk flow...
