There are few things to consider here : (Before starting KINDLY note - His Present Weight must be about 12.5-12.8 kg ; Height must be about 86-88 cm as per his age - 2 years 1 month) : (1) Average daily consumption of milk in general (Which must not exceed 250 to 300 ml per Day). Also , to make sure his milk consumption must be after morning breakfast or Preferably in the afternoon or evening. (2) Second Important Question would be whether he consumes more of water or milk in total before or during meals!!! Which must be certainly changed. And the timings can be after 20 to 30 minutes at least of food intake with initially only small amount taken (immediately) after food. With a total of 100 ml per kg (if your child is 10 kilos then about 1 Litre) per day. If beyond 10 kilos then every extra kg added after 10 must be multiplied by 50 (applied upto a total weight of 20 kgs only) : eg., 12.5 kg child ---> ( first 10 kg X 100 - 1000 ml ) + (next 2.5 kg X 50 - 250 ml) ---> Total 1250 ml per day (May add other juices too but as you mentioned he doesn't like then or he spits or vomits probably). (3) If his vomiting is actually a true vomiting after about 10 to 15 minutes of giving the fruit juice and not immediately (in which case it is only spitting because of not liking) then we need to make sure he gets water and watery juices and other sources in small amounts throughout the day as he may have something called as Reflux which is quite common in children in general but if that is hampering the growth of the child (Weight being same for many months or increasing very slowly) then we need to manage it methodically . Firstly if this is only for certain items which he doesn't like or in general after Consuming large amounts of liquids in general then to see a Pediatrician in OPD to make sure these points correlate and if required to provide medications and if necessary to conduct few tests too. But either way just taking care of the fact that ONLY SMALL amounts of liquids to be given at a particular time will partly reduce this issue along with reduction of fluids in general (esp Milk if he is taking in excessive amounts like 500 ml or more per day). As milk is known to be not useful beyond 250-300 ml per day after 1 to 1.5 years of age and in turn may sometimes predispose a child to Constipation or Anemia. (3) It is always advisable to start his day with high protein rich diet (NOT milk) like boiled eggs plus regular breakfast that everyone eats at home with avoidance of water intake in between. (4) Also to consult to your Pediatrician to know if he has been started on anti-helminth medication (given as Prophylaxis against Worm infestation every 6 months). (5) To ensure his daily routine in passing stools is also maintained and voluntary with he fact that he passes it mostly in the morning and not in odd hours or changing every day which can predispose to further constipation (along with milk) called habitual constipation. (6) To ensure he has every meal with rich pulses and moderate carbohydrates and fats , especially in the breakfast followed by lunch and dinner. Avoid junk foods at all costs. Avoid fizzy drinks at all costs. (7) To note if he has tendencies for runny nose more often than others - more than 6 to 8 times per year or once every month. This denotes allergic tendencies and hence he needs to be managed accordingly as per your Pediatrician's plan. (8) Always maintain a healthy diet plan for him and any deviation of the same for the want of junk foods must be discouraged or given in very small amounts ONLY after they complete their regular food. (9) If he happens to be a sole child then to make sure others in the house especially grand-parents and including parents , to NEVER give away to their demands or pamper them for the want of chocolates and junk foods or bakery items just because they are asking. They NEVER know what is right or wrong for themselves at this point of time. And hence every parent must take this as an opportunity to first let then know that this is not good for them and as most children may not listen then to make them have a MANDATORY complete meal followed by the small amount of Junk food. To make sure the other relatives around comply to this Basic rule of Yours. (10) To always try to provide variety in the foods to make sure children don't get bored especially the breakfast and lunch parts in order to make sure they grow and develop well. Make sure to comply with the nutrition advise from Your Pediatric Doctor and in case You are following the advise of a fully Trained Pediatric Nutritionist then to consider those as well. Best of luck in your endeavours to make your child healthy. Also to make sure Your Pediatrician charts your child's height ; weight and weight for height in the IAP - WHO recommended GROWTH CHART (Revised 2015) and plan his diet and nutrition accordingly. Take care.
