Bleeding & swollen gums due tartar and plaque acumulation between gums and teeth ..
Need clinical evualtion followed by Deep cleaning ( Scaling ) under local anesthesia followed by mouth rinses and Medication . Correct brushing technique.

If the swelling and bleeding is in particular area, then it might be due to 2 reasons
1. Local gum trauma due to harsh brushing, pricking with pin or tooth pick
2. Might be decay( caries ) in that particular tooth
If the swelling and bleeding is all over the gums, then you might need a professional cleaning.
Professional cleaning is advisable twice a year, if needed.
