May be you have abdominal TB it's a commonly seen in those who have findings of mesenteric lymphadenopathy that's one findings in your ct report and other else finding you also have small umbilical hernia with fat content it is insignificant when patient not experiencing pain over there if it is there that means inflammatory changes going there then you need surgical hernioplasty but before that you should go for physical Examination for further evaluation
