My sister doesn't talk well. She has so many fears like height, steps. She is unable to adjust with new people. She doesn't know how to behave or how to protect herself properly. Most of the things she want help of my mother.

You have mentioned down's syndrome in the subject. I want to know if she really has down syndrome or you think it is.
Down's syndrome child is a special child and they do need assistance.
I woukd suggest you take her to a general physician in person for further evaluation and management.
