Perforation of ear drum which is not healing spontaneously and associated with significant hearing loss needs to be treated by surgery known as tympanoplasty. Medicines are usually not beneficial. Cost of surgery depends on the doctor and the hospital. It may cost anywhere between 30k to 70k
