After almost a year, with certified medical attention for HIV, Hep -V and C, she had direct sex with coconut oil without using condoms and it's causing vaginal pain and Fever.

Apparently she didn't have sexual intimacy for almost a year. That's why the pain. She should use lubricants like coconut oil or Lubic cream. It should be gentle sex. She will overcome these symptoms.
Conception is a problem when you have to use any lubricants. So you should plan for pregnancy at a later date.
