For loose motions give digestive enzyme preparation as Aristozyme dps.   Rashes are due to rubbing everytime while cleaning the baby as baby skin  is very soft nd delicate.
Next Steps
so better mop the diaper area very gently with slighly warm water. When dreid apply EUMOSOME ointment

Newborn babies may pass frequent loose golden yellow  stools with granules in it and is normal...no medicines required... Feeding from one breast at a time, and next time from the other breast with proper latching of the baby over areola of the breast would help baby getting fore milk and hind milk...with proper feeding technique the frequency of stools may reduce... You can apply candid cream around butt area to reduce the rashes after every stool or urination...do not use wipes for cleaning...use warm water instead..
