Early childhood caries usually can be seen in children aged between birth to 71 months,
You need to visit a dentist, you may require topical fluoride application, restoration and instructions to maintain oral hygiene,
Brushing twice daily has to be continued

He must be bottle feeding and what ur looking at might me rampant caries. You might be brushing twice but pouching of food, sweets, toffees all lead to Thai. I suggest you to visit a pedodontist to take care of these teeth as well as give preventive therapy to other teeth by protecting them from further caries attack.
