We understand your worries, I would like to have a pictorial view of the issue as it’s difficult with the way u are trying to describe .
And I would like to know a more proper history about,if he is trying to show any other symptoms other than this.
If he doesn’t have any other symptoms, please do not disturb the skin,leave it and u can go and get him examined after things get settled down.

Please consult paediatrician. If it is causing obstruction to void urine, or repeated urine infections or any other symptoms, paediatrician may suggest surgery
