PFPS. Burning pain.
I have been suffering from pfps, patella subluxation for past 9 months. I didn't injure anywhere. It started off with a new walking routine which I did on the hilly roads. Knee burning pain started off on top and right side of the knee(right). I have swelling on the same spot. Doing PT for past 6 months. No changes in my symptoms. Did aggressive dry needling sessions. Nothing helps. Doing home exercises along with foam rolling for quads. Not going to PT since lockdown. The burning pain is more since last week along with the increase in the swelling. It burns whenever I bend, stand, stretch my legs straight while sleeping. So it's a constant pain throughout the day irrespective of what I do. What to do to get better? Is it normal to have burning issue and swelling for so long. I had 2 MRIs. MRI says lateral Patella subluxation, chrondomalacia patella, congenitally/developmentally prominent patella alta and lateral trochlear ridge.What to do to get better from this constant knee burning?

Chiropractic adjustments will help Contrast Fomentation (Hot and Cold ) Avoid Squatting- Avoid sitting Cross legged. Quadriceps strengthening exercises- Quad Clenches: Lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, Hold for 3 secs. Repeat 10 times twice a day. Short Arcs: Lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day.

Longtsanding Patellofemoral issues can be related to your foot alignment which leads to fauly walking pattern.. This can be evaluated with your walking analysis.. If there is any issue with feet it can fixed with orthtics and proper sports shoes.. In exercise add foam roller for IT band and VMO strengthening

Patella subluxation takes about 8 weeks to heal ...if it is more than 9 months and symptoms are still the same may be u have not taken proper care as it have chances to reoccur again For swelling,apply cryotherapy for around 15 mins twice a day Exercises 1)standing hamstrings stretch by putting the heel Of your injured leg on stool 2)quads sets which u are already doing as mentioned 3)straight leg raise 4)side lying leg lift by lying on your uninjured site

Hi, Continue simple pain free exercise and use good quality knee brace. But Unfortunately conservative treatment is not working as your knee pain has become chronic. Try to rest .

First to reduce your pain you should give Cryotherapy or Ice therapy for 15-20 minutes every four hours for one week. You can use Volini gel or spray to the site provided you are not allergic. You should do hip strengthening and knee strengthening exercises. You should do stretching for your IT band. You should do stretching for your hamstrings and calf muscles. You should do K - taping for your knee to maintain the correct position.
Next Steps
Avoid floor sitting. Avoid full squat. Avoid standing for long time. Keep changing your knee position. Avoid cross leg sitting.

The issue you are suffering from is not just that pain and burning but is rather multifaceted. It is a tightness and weakness of your IT Band and tensor Fascia Lara which are are the fibro muscular bands in the outer side of your leg. This is leading to compression of the nerve in the outer aspect of thigh which leads to a condition called meralgia perasthetica. It’s not only the dry needling, etc that will help rather it’s a graded exercise program for the complete knee musculature aimed at strengthening the inner side of the thigh and at the Sam Ernie stretching out the tight structures on the outer side which are leading to patellar subluxation and burning pain. This will ultimately give you a permanent relief in your symptoms.

I wouldn’t treat a PFPS patient with aggressive dry needling as that is only addressing the tip of the iceberg. Physio has to check for VMO weakness, tracking the patella back . But since PT I discontinued due to lockdown, have you tried continuing just cold pack and basic knee exercises including knee isometric exercises for VMO?

Apply ice for pain. Dont do exercise till the pain subsides. Visit to nearest physiotherapy clinic to avoid complications.
