This is most likely to be balanitis. It can be a fungal or bacterial infection. Treatment for the same needs to be started after examining it, better by a skin specialist. To prevent it in future you need to keep your sugars under control. You medicines also need to be reviewed. Some antidiabetic medicines do cause balanitis
