Hi. You can use a desensitising tooth paste for the sensitivity,it can be helpfull to some extent. Basic cleaning of teeth can be useful and you need to consult a dentist as soon as possible before the condition further worsens.
Brush your teeth last thing at night and at least one other time during the day, with fluoride toothpaste. Consider using toothpaste specially designed for sensitive teeth. Use small, circular movements with a soft- to medium-bristled brush. Try to avoid brushing your teeth from side to side.
Change your toothbrush every two to three months, or sooner if it becomes worn.
Don't brush straight after eating - some foods and drinks can soften the enamel of your teeth, so leave it for at least an hour before you brush.

The Teeth having different layers, the outer layer is the protective layer which we called enamale which protect the second layer dentin which having nerve endings of the pulp the vital aspect of teeth, now in time process ( with age) or some pathology the enamle was lost causes opening of this nerve ends which got exposed to the hot,cold you take and causes problems.
Now about the solution, there is no home remedy is not know to Dental literature was mentioned or even these sensitivity having no specific treatment is available except symptomatic relief which is only possible when you can consultant with someone.

You can use a Tooth paste (any desensitising paste available) which needs to be applied on the tooth surface using finger tips, leave it on for 2 minutes and rinse it off. It will takw some time for the sensitivity to subside. You can use it twice in a day.
