Two episodes of syncope with sweating and hypotension with blood pressure of 98/49 mmhg on first episode and 98/60 mmhg on second episode. Patient compliance for medication is good. Good glycemic control.
What should be the reason for present condition, how to manage if next episode occur?

if he having such low blood pressure i do not understand the point of taking the mentioned anti-hypertensives. kindly look into that aspect.
the antihypertensives which he is taking are -metoprolol, telmisartan, chlorthalidone. also isosorbide mononitrate also has blood pressure lowing property. consult a cardiologist for further assistance.
if his blood pressure is normal without the medications, then he just needs to make some dietary changes like salt restriction and avoiding oily and spicy foods.
