As you have mentioned that you have upper back , neck and shoulder and your pain is radiating to the hands .. due you have a desk job..
If so you should try to maintain a correct posture while working..
Pls keep your back as straight as possible maintaining the hollow in the back and also try to chin tuck exercises..
