Dear practo user I could understand the pain you are going through. There are some clinical test to confirm verical fracture of tooth if it's present. Don't be hurry for treatment. Get opg , full mouth x-ray done to rule out any other issue . Rather extraction even fracture tooth can have possibility of treatment and can be saved. Can share that too.

It is neither your upper 1st molar, neither lower 1st molar. It could be your upper 3rd molar. Get your cheeks tested for marks of cheek bite as well,  some ulcerations or white patch marks. Was the lower wisdom tooth removed anytime before? Send a complete X-ray if possible.
Health Tips
Do not remove 1st molar teeth at all before confirming the diagnosis
